FULL STEPS: How to generate e-affidavit for change of name, loss of SIM, others

The Lagos State judiciary has officially digitised its affidavit processes with the introduction of electronic affidavits, allowing residents to swear and submit statements online via the Lagos State Court Management Information System (Lagos CoMiS).

This initiative, designed to streamline court procedures and enhance efficiency, enables affidavits to be created, signed, and notarised digitally in compliance with legal standards by authorised judiciary personnel.

According to a recent statement by Kayode Agbedejobi, Esq., Publicity Secretary of the Nigerian Bar Association (NBA), Ikeja Branch, starting June 1, 2025, all general affidavits — including Change of Name, SIM Loss, Declaration of Age, and similar documents — must be processed exclusively through the Lagos CoMiS platform: https://lagoscomis.lagosjudiciary.gov.ng.

However, affidavits used for litigation at the High Court are excluded from this directive and will continue under the current manual process.

Court fees for e-filing can be paid using debit/credit cards and other supported payment channels.

Here are the steps below:

• Open a browser and dial the LagosCoMiS website – https://lagoscomis.lagosjudiciary.gov.ng
• Enter the valid “Legal Email Address’’ and ‘Password’. Click ‘Sign in’
• On your Dashboard, click on ‘New Filings’ on the top right corner of your page.
• Select affidavit of choice from the available list or general affidavit for a customized version
• Complete the form displayed with accurate information, upload passport photograph and Click “Create Affidavit” different paragraphs will appear, you can edit them by clicking on the three dots on the right side of each of them. Click “Paragraph”, “Sub Paragraph” or “Sub Sub Paragraph” to input additional information.
• Make sure you click the icon after inputting so as to save.
• N/B: If it is a general affidavit, you will type these paragraphs yourself
• Click “Preview and Proceed”
• If you notice any error, click ‘Cancel’ to go back and make the necessary corrections, else Click ‘Done’
• You will get a notification that your affidavit was successfully saved, click ‘Ok’
• Enter your full name, email address, and phone number, then Click ‘Proceed to Payment’ to make payment.
• Click “Upload Video” to upload deponent identification videos, and “Upload Signature” to upload eSignature.
